**The Foot Locker BJJ Waiver and Release of Liability**

I, acknowledge that I am voluntarily participating in Brazilian Jiu-Jitsu classes and related activities at The Foot Locker BJJ club. I understand that Brazilian Jiu-Jitsu is a full-contact martial art that involves physical contact, grappling, and other strenuous activities that may result in injuries.

In consideration for being allowed to participate in classes and events at The Foot Locker BJJ, I agree to assume all risks and responsibilities for any injuries or damages that I may sustain as a result of my participation. I understand and acknowledge that injuries are a common and inherent risk in Brazilian Jiu-Jitsu, and I release The Foot Locker BJJ, its coaches, instructors, staff, and affiliates from any and all claims, liabilities, demands, and causes of action that may arise from my participation in classes and events.

I understand that it is my responsibility to inform the club's instructors and staff of any physical limitations, injuries, or conditions that may affect my ability to participate safely. I agree to follow all safety guidelines and instructions provided by the club's coaches and staff during classes and events.

I understand that this waiver and release of liability is a binding legal agreement and that by signing it, I am giving up my right to bring a legal action or assert a claim against The Foot Locker BJJ, its coaches, instructors, staff, and affiliates for any injuries or damages that may occur during my participation in classes and events.

I have read this waiver and release of liability in its entirety and fully understand its contents. I voluntarily and knowingly agree to its terms by signing below.

**Photo Consent Form**

I, hereby grant permission to "The Foot Locker BJJ" and its associates to take photographs and videos of me during my participation in club activities, including but not limited to training sessions, competitions, events, and related activities.

I understand and agree that these photographs and videos may be used by "The Foot Locker BJJ" for promotional purposes, including but not limited to social media platforms (e.g., Facebook, and Instagram), the club's website, marketing materials, and other promotional channels.

I hereby release and hold harmless "The Foot Locker BJJ," its representatives, and all persons acting under its permission or authority from any liability that may arise from the use of such photographs and videos.

I understand that these photographs and videos may be accessible to the public and may be shared and distributed beyond the control of "The Foot Locker BJJ." I agree that I will not hold "The Foot Locker BJJ" responsible for any misuse or unauthorized distribution of these photographs and videos by third parties.
